Background: Patellofemoral osteoarthritis (PF OA) is exceptionally predominant and limiting. However, little is known about the risk factors that contribute to its onset and progression. Purpose: The aim of this study was to decide if women with PF OA descend stairs using different muscular activation strategies compared to similarly aged healthy controls. Methods: Thirty-one women with isolated PF OA and 11 similarly aged healthy women took part in this study. The activation onset and duration of PF OA in vastus medialis oblique (VMO), vastus lateralis (VL), gluteus medius (GM), transversus abdominis (TrA), and multifidus muscles were evaluated during the stair descent task using surface electromyography (EMG). Results: There was a non-significant difference between women with PF OA and healthy controls regarding all tested variables, except for the GM activation onset that was significantly delayed in women with PF OA, with the p-value of 0.011. Conclusion: The causes of PF OA differ and might not always be due to a lack of quadriceps strength or VMO activation deficiency, and prospective longitudinal studies are required to confirm this assumption.

Background: Exotropia control is deteriorated by post-strabismus surgery in many cases. Improving this control is considered as an important factor for success of the strabismus surgery like ocular motor alignment. Objective: To determine the therapeutic effect of postoperative eye exercises on exodeviation eye control after the strabismus surgery. Methods: Forty patients suffering from intermittent exotropia after strabismus surgery were randomly divided into experimental group and control groups. Both groups received the usual medical care, while the experimental group received different types of eye exercises using physiotherapy modalities in 24 sessions over three months (two sessions a week). The measurement of exotropia control for near target and far target using the office-based scale was done before and after the treatment. Results: There was a significant improvement in near eye control post-treatment (0.45±0.61) in the experimental group compared with control group (3.65±0.88) as the mean difference was -3.20 and 95% CI of the difference between them was -3.645--2.755 with P<0.01. Also, far eye control improved post-treatment in the experimental group (0.75±0.64) compared with control group (4.00±0.86) with mean difference of -3.250 and 95% CI of the difference between them was -3.727--2.773 with P<0.01. Conclusions: For patients with intermittent exotropia who had undergone strabismus surgery, the addition of physiotherapy exercises for the eye provided significant improvements in exotropia control for near target and far target.

BACKGROUND: In light of the global commitment to promoting sustainable dietary choices and the pivotal role of young adults in the adoption of these choices, it is paramount to understand their perspectives of healthy and sustainable diets. The present study aimed to investigate the validity and reliability of a questionnaire examining the knowledge, attitudes, practices, and willingness to change regarding sustainable diets among young adults in the United Arab Emirates (UAE). METHODS: Male and female students attending University of Sharjah, UAE (n = 436), completed an online questionnaire consisting of four sections: knowledge, attitudes, practices, and willingness to change with regard to sustainable diets. Of the participants, 106 completed the questionnaire a second time, 1 month later. Factor analysis (FA) (both exploratory and confirmatory), Cronbach-α, interitem correlations, and intra class correlation (ICC) coefficients were used in the data analysis. RESULTS: Exploratory FA revealed four factors corresponding to the components of the questionnaire. Results of the confirmatory FA indicated a good fit: the χ2 df ratio was < 5 (2.3), root mean squared error of approximation was < 0.08 (0.048) and the comparative fit index was above 0.9 (0.901). Cronbach α and interitem correlations were: knowledge: 0.57 and 0.21; attitude: 0.70 and 0.28; practices: 0.76 and 0.39; willingness to change: 0.69 and 0.27. The ICC coefficients, assessing the reliability of the questionnaire, ranged between 0.48 and 0.92 for the various items. CONCLUSIONS: The developed questionnaire is a valid and reliable tool that could be used to identify the gaps and opportunities for the development of evidence-based interventions aiming to enhance the uptake of sustainable diets among young adults.

DESIGN: Single-blind, two-arm parallel-group randomised controlled trial. CASE SELECTION: Patients aged 11-14 years old attending for comprehensive orthodontic treatment between January and July 2018. All subjects needed to have the following: present upper first premolars and first permanent molars; transverse maxillary deficiency; unilateral or bilateral posterior crossbite. The exclusion criteria was: cleft lip or palate, previous orthodontic treatment; congenital deformity; absent permanent teeth. INTERVENTION: Rapid maxillary expansion via two techniques were used and placed by the same orthodontist. Group A were treated via the tooth-bone-borne Hybrid Hyrax expander, Group B had the tooth-borne (hyrax) expander. CBCT scans of the maxilla were taken before treatment and 3 months after the activation phase when the appliances had been removed. DATA ANALYSIS: Analysis of the dental and skeletal changes was completed via measuring pre and post treatment CBCT scans using Dolphin software for Group A and Group B. Measurements included: specific naso-maxillary widths in the first premolar region (i.e. nasal cavity, nasal floor, maxilla, palate), naso-maxillary widths in the first molar region, premolar/molar inclination, buccal cusp distance, and apices distance, as well as suture maturation. Baseline characteristic data was compared using one-way ANOVA. Intergroup comparison of changes was analysed using ANCOVA. P < 0.05 (5%) was considered statistically significant. Inter-rater reliability was assessed using correlation coefficient. RESULTS: Skeletal effects: The Hybrid Hyrax patients (HHG) showed a significantly higher increase in dimensions in the nasal cavity, nasal floor and maxilla in the premolar region only, of 1.5 mm, 1.4 mm and 1.1 mm, respectively, compared to the Hyrax expander patients (HG), to a 5% significance level. The HHG showed a significantly higher increase in dimensions in the nasal cavity of 0.9 mm, in the molar region, compared to the HG. Dental effects: Premolar inclination was significantly higher in the HG, with the difference being -3.2 degrees on the right 1st premolar and -2.5 degrees on the left 1st premolar. The higher the amount of activation, the higher the nasal skeletal changes in the Hybrid Hyrax group. CONCLUSIONS: The Hybrid Hyrax (tooth-bone-borne expander) resulted in increased skeletal dimension changes (i.e. in the nasomaxillary structures in the first premolar region, and nasal cavity in the first molar and first premolar region) and only minimal premolar inclination/tipping compared to the Hyrax (tooth-borne expander). There were however no differences between the expanders for position of premolar or molar apices, or molar crowns.

RESUMO

We describe the case of a 68-year-old woman who was referred to the Oral Medicine Department in the Bristol Dental Hospital in November 2018 regarding a mass in the hard palate. The patient was previously diagnosed with stage IV mantle cell lymphoma of the upper tarsal conjunctiva in December 2014. Further investigations revealed lymphomatous deposits in other sites throughout the body. This patient underwent six cycles of chemotherapy, followed by consolidation LACE autograph and maintenance rituximab. While mantle cell lymphoma very rarely presents on the hard palate, knowledge of its clinical features and differential diagnoses is imperative in its accurate diagnosis and appropriate management.

Objective The main purpose of this article is to determine the frequency of neurotologic complications after posterior fossa microvascular decompression (MVD) surgery. Design Retrospective case review. Setting Tertiary care referral center. Participants A total of 215 consecutive MVD operations by a single surgeon between March 1996 and May 2016 were reviewed with 192 surgeries on 183 patients meeting inclusion criteria. Main Outcome Measures Neurotologic complications secondary to MVD. Results The 52 males and 131 females had a mean age of 58.52 years (range, 28-92 years). Indications for MVD were trigeminal neuralgia ( n = 162), hemifacial spasm ( n = 23), glossopharyngeal neuralgia ( n = 13), vagal palsy ( n = 1), and tinnitus ( n = 1). The outcomes examined were postoperative hearing loss, tinnitus, vertigo, and hemifacial paresis. At least one of these complications was present in 17.7% of patients. There were 4.17% with permanent hearing loss, 6.77% with transient hearing loss, 5.21% with tinnitus, 5.73% with vertigo, and 0.52% with hemifacial paresis. There was no significant difference in complication rates based on surgical indication. Conclusions Neurotologic complications are a significant risk when performing MVD. It is important for otolaryngologists as well as neurosurgeons to be aware of such complications. We recommend perioperative audiometry in all patients undergoing MVD and believe there is utility in routine otolaryngologist involvement.
